Due to the limited number of available appointments, a cancellation fee of $150 is applied to cancellations made within 48 hours of your appointment time. No-Show fee for a previously scheduled appointments is $250.
If at any time prior to your appointment you experience any common COVID-19 symptoms such as fever, cough, shortness of breath, loss of smell or taste, or come in close contact with anyone who has COVID-19 or is quarantined, please obtain a PCR test prior to your appointment. In the event that this test result is positive, please contact us to reschedule your appointment without a penalty.
We are an ‘out-of-network provider’ for all dental insurances. We would submit all insurance claims to your dental insurance. You are financially responsible for the portion of the fee not covered by your dental insurance.*
*Full pre-payment is required for all cosmetic treatments, orthodontic aligners and oral appliances. All other fees are due upon appointment confirmation. Late payments are subject to a service fee of $25.
